Forms to Download

As per FHSAA rules, every student athlete must have a sports physical. Sports physicals are good for one year from the date of when the physical was taken.

FHSAA Athletics Physical Form

Each participant must have a parent permission form on file before the student is allowed to participate. Both copies are needed and this form must be notarized.
